What's in a Driver Qualification File? The FMCSA Checklist

Published October 21, 2026

A Driver Qualification File is one of the first things an auditor asks for. Here is exactly what FMCSA requires in each DQF and how Test Pro keeps yours complete.

When the FMCSA audits a carrier, the Driver Qualification File (DQF) is one of the first things they check. A missing document can mean a violation. Here's what belongs in every DQF.

What a DQF Is

A Driver Qualification File is the set of records that proves a driver is legally qualified to operate your commercial vehicles. You must keep one for every driver, and keep it current for as long as they drive for you plus three years after.

The FMCSA DQF Checklist

At minimum, each driver's file should contain:

  • The driver's employment application
  • A copy of the CDL and any required endorsements
  • The Motor Vehicle Record (MVR) from the time of hire
  • An annual MVR review and a list of violations the driver certifies each year
  • The road test certificate or an acceptable CDL equivalency
  • The current medical examiner's certificate (and CDL/medical self-certification)
  • Safety Performance History from previous DOT employers (past three years)
  • Clearinghouse pre-employment query, plus annual queries

Where Carriers Get Tripped Up

The most common DQF problems we fix are expired medical cards, missing annual MVR reviews, no previous-employer safety history, and a skipped Clearinghouse query. Any one of these can turn into a violation during an audit — and they're all avoidable with a system that tracks expiration dates.

How Test Pro Keeps Your DQFs Audit-Ready

We build a complete DQF for each of your drivers, run the MVR and Clearinghouse queries, collect the previous-employer safety history, and track every expiration so nothing lapses. When an auditor asks, your files are complete and current. Hire a new driver and we set up their file from day one.

Frequently Asked Questions

How long do I keep a Driver Qualification File? Keep each DQF for as long as the driver works for you, and for three years after they leave.

What are the most common DQF violations? Expired medical certificates, missing annual MVR reviews, no previous-employer safety history, and skipped Clearinghouse queries are the issues we see most often.

Can Test Pro manage DQFs for all my drivers? Yes. We build and maintain complete, audit-ready Driver Qualification Files for your whole roster and track every expiration date for you.
